Problems and precautions
The health concerns to discuss before a plan
Use the medicine’s official warnings to prepare a conversation about your own history and the help available after treatment.
Common stomach problems
The label, or official medicine information, for tirzepatide (Zepbound) lists these common problems in September 2026: feeling sick to the stomach, loose stools, throwing up and constipation, or trouble passing stool.
In its weight trials, nausea affected 25% to 29% of people receiving Zepbound, compared with 8% receiving placebo, a dummy shot.
The range comes from the different treatment groups in the trials.
Stomach problems can be severe, and fluid lost through diarrhea or vomiting has been linked with kidney injury.
The label does not recommend Zepbound for severe gastroparesis, which means very slow stomach emptying.
The thyroid finding in rats
Tirzepatide’s labels put their strongest FDA warning in a box around the finding of thyroid tumours in rats.
Whether tirzepatide causes these tumours in people is unknown.
Medullary thyroid cancer is the name of a rare cancer of the thyroid. A history of it in you or your family rules out tirzepatide under the labels. Having the rare inherited illness MEN 2 also rules it out. The longer name for MEN 2 is multiple endocrine neoplasia type 2; inherited means passed from parent to child. Ask for help checking family records if a diagnosis is uncertain.
A past serious allergy to tirzepatide or any other product ingredient also rules out use under the labels.
Pregnancy and birth control
The September 2026 Zepbound label says to stop Zepbound when pregnancy is recognised.
The label tells prescribers to advise people taking birth control pills to switch to a method that is not a pill, or add a barrier such as a condom. It gives the same time span for either choice: 4 weeks after starting and 4 weeks after each dose increase.
Ask how these label statements fit your plans for pregnancy and birth control.
Other care needs to be in the picture
Taking insulin or certain diabetes pills alongside tirzepatide can cause low blood sugar, the labels warn. They also warn that eye disease already caused by diabetes can get worse. For surgery or another procedure, there is a different concern: stomach contents can enter the lungs under general anesthesia or deep sedation.
Anesthesia and deep sedation are medicines used to put a person to sleep or keep them very drowsy for a procedure.
The labels do not settle whether stopping before surgery prevents that lung problem.
Ask the surgical team and prescriber how they will coordinate your care.
Severe symptoms need urgent help
Serious allergies have been reported with tirzepatide. Reports also include pancreatitis. This means painful swelling in a belly organ called the pancreas. Pain that will not stop, or is severe, is a warning sign; it can spread to the back.
For severe allergy symptoms, MedlinePlus gives an urgent direction: call 911 or seek emergency care. Examples include trouble breathing and a tongue that swells.
For belly pain that is severe or will not stop, contact the prescriber now for medical help. Do not save it for a routine follow-up visit.
Ask who to call if you feel unwell. Ask what needs a call now and what can wait. Get that plan in writing before you leave the visit.
